- Student Recovery Contract
- Parent Contract
- Consent Form to Obtain/Release Information
- Psychological Educational Evaluation completed within the last two years
- Most recent IEP (if applicable)
- Most recent Psychological Evaluation
- $100 non-refundable application fee (Check should be made out to The Bridge Way School)
